Getting your DD214/NGB22

Our office encourages Veterans who need to request their DD214 Form to visit the Warren County Veteran Services located at 320 E. Silver St., Lebanon, Ohio, 45036.

The Warren County Veteran Services will not only submit the form to the National Archives and Records Administration (NARA), but will also arrange for the DD214/NGB22 to be mailed directly to the Veteran.

Working with Veterans Services will ensure that Warren County’s Veterans will be informed of all available benefits afforded to the veterans’ community when they request their DD214/NGB22.

This service is provided to Veterans free of charge.

Your request must contain certain basic information to locate your service records. This includes:

  • The Veteran’s complete name used while in service
  • Service number
  • Social security number
  • Branch of Service
  • Dates of Service
  • Date and place of birth
  • If you suspect your records may have been involved in the 1973 fire, also include: Place of discharge, last unit of assignment, and place of entry into the service, if known.
